Agreement and Authority for Payment and/or Direct Debits

I acknowledge that I have been given the option of choosing a membership based on either periodic fortnightly billing or prep payment.

I acknowledge that if paying by Direct Debit I need to give 30 days notice in writing for it to be cancelled and seven days to suspend. I can accrue up to five lessons at any one time but these must be used by end of Term 4 and these do not carry over to the following year. If paying yearly a 10% discount only applies if paid in full by no later than 28 February 2010 or full fees will apply. All Lesson Packs are to be paid in advance before classes commence as do other payment options. Insurance must be paid by 28.02.10 or a late fee of $20.00 will be charged for Term 1 and all new members from Term 2, Insurance is payable on sign up. I understand that I will not be able to train until all my fees are paid and up to date.

I also agree as a Member of Hawkesbury Martial Arts, if over the age of 18 years to become a member of the Richmond Club as part of our agreement with them as our Major Sponsor for 2010. ^[1]

Affiliation fee and insurance: Fee covers: School personal accident insurance, the right to train at any KBI Centre for the term fee, affiliation to the New South Wales Karate Association, Australian Karate Association, World Karate Association. The opportunity to train at sister schools in India, the USA, Norway, Wales, Pakistan, UAE, Sri Lanka, Malaysia, Germany and across Australia.

Insurance is based on which Term you join: Term 1 - $55     Term 2 - $45     Term 3 - $35     Term 4 - $25
